Se’if 1א׳
סֵפֶר תּוֹרָה שֶׁאֵינוֹ מֻגָּהּ, אָסוּר לְהַשְׁהוֹתוֹ יוֹתֵר מִשְּׁלֹשִׁים יוֹם, אֶלָּא יְתַקֵּן אוֹ יִגְנֹז. וְהוּא הַדִּין לִשְׁאָר סְפָרִים (רַשִׁ"י וּבֵית יוֹסֵף בְּשֵׁם הַגָּהוֹת מַיְמוֹנִי וְרַבֵּנוּ יְרוּחָם נָתִיב נ'). וְאֵין לְהַגִּיהַּ שׁוּם סֵפֶר עַל פִּי הַסְּבָרָא, כִּי אִם בִּרְאָיָה בְּרוּרָה שֶׁיֵּשׁ בּוֹ טָעוּת (הָרַב ר' דָּוִד כֹּהֵן בַּיִת ט"ו וְחֵרֶם קַדְמוֹנִים).
A Torah scroll that has not been corrected — it is forbidden to keep it more than thirty days; rather it shall be repaired or stored away. And the same law applies to other books (Rashi, and Beit Yosef in the name of Hagahot Maimoni and Rabbeinu Yerucham, netiv 50). And one should not correct any book on the strength of reasoning, but only by clear proof that there is an error in it (Rabbi David Kohen, bayit 15, and the ban of the early authorities).

קָרְאוּ בְּסֵפֶר תּוֹרָה בְּצִבּוּר וְנִמְצָא בּוֹ טָעוּת, מוֹצִיאִין סֵפֶר תּוֹרָה אַחֶרֶת וּמַתְחִיל מִמָּקוֹם שֶׁנִּמְצָא הַטָּעוּת, וּמַשְׁלִימִין שִׁבְעָה עַל אוֹתָם שֶׁקָּרְאוּ בְּמֻטְעֶה. וְאִם נִמְצָא הַטָּעוּת בְּאֶמְצַע קְרִיאַת הַתּוֹרָה, גּוֹמֵר קְרִיאָתוֹ בַּסֵפֶר הַכָּשֵׁר וּמְבָרֵךְ לְאַחֲרֶיהָ וְאֵינוֹ מְבָרֵךְ לְפָנֶיהָ, כִּי בְּרָכָה שֶׁבֵּרַךְ לְפָנֶיהָ בַּסֵפֶר תּוֹרָה הַפָּסוּל, וְכֵן קְרִיאָה שֶׁקָּרְאוּ בּוֹ, עָלְתָה לָהֶם בְּדִיעֲבַד. הַגָּה: וְאֵין חִלּוּק בָּזֶה בֵּין שֶׁנִּמְצָא הַטָּעוּת בְּסֵפֶר תּוֹרָה רִאשׁוֹנָה בֵּין שֶׁנִּמְצָא הַטָּעוּת בַּשְּׁנִיָּה שֶׁקּוֹרִין בָּהּ חוֹבַת הַיּוֹם לְמַפְטִיר וְעַיֵּן בא"ח סִימָן קמ"ג.
If they read in a Torah scroll in the congregation and an error was found in it, they bring out another Torah scroll and begin from the place where the error was found, and they complete the seven, counting those who read in the faulty one. And if the error was found in the middle of the reading of the Torah, he finishes his reading in the fit scroll, and blesses after it and does not bless before it; for the blessing that he blessed before it over the unfit Torah scroll, and likewise the reading that they read in it, counts for them after the fact. Hagah [Rema]: And there is no distinction in this between where the error was found in the first Torah scroll and where the error was found in the second, in which they read the obligation of the day for the maftir (the one who concludes the reading). סֵפֶר תּוֹרָה שֶׁנִּמְצְאוּ בּוֹ שָׁלֹשׁ טָעֻיּוֹת, אָסוּר לִקְרוֹת בּוֹ עַד שֶׁיַּגִּיהֶנּוּ, כִּי הוּא מֻחְזָק בְּמֻטְעֶה.
