There are 5 lobes (like ears) in a lung, 3 on the right and 2 on the left from the perspective of the animal hung from its feet and its intestines are towards the butcher; such that the right side of the animal and the right side of the butcher are next to one another. On the right, there is another small lobe called עינוניתא דורדא (rose-like lobe, access lobe) and is not in line with the other lobes, rather is further and close to the middle. If one lobe is missing from the right or left, or if there are 2 to the right and 3 to the left, it is teref. RAMA: The figure of the right lobes differ from the figure of the left lobes and if they switch places, the animal is teref. Also, if the middle right lobe is larger than the bottom, the animal is teref. If the upper left lobe is larger than the upper right lobe, the animal is teref. If the right large lobe is larger than the that on the left it is called "switched" and the animal is teref (as per the Maharal of Prague). SHULCHAN ARUCH: If the access lobe is missing, doubled, or on the left, it is kosher. RAMA: In these countries there is the custom to taref whether the access lobe has 2, is missing, or in another place. That is what we follow since the majority hold that way and we don't change customs. If it changed from its normal form or shape, it is taref. If there are 2 access lobes with a single bronchus connecting them the size of a finger, even if there is a crack it is kosher. If one is reversed and the second isn't we go according to the larger one. If the larger one is not reversed, it is kosher. The access lobe tends to be in a pocket. If it is not found in a pocket, there are 2 pockets, it is outside of its pocket, when placed inside it pops out when inflating the lung, it is taref. This is only the law in places that taref with 2 or no access lobes, but in places that allow, all of the above are also allowed. If part of the access lobe is missing, check on the left lobe or large lobe. If the missing piece is found there, some say it is kosher since is sometimes found in goats or lambs. Some allow if its base looks like an access lobe and is the thickness of a finger even though the head doesn't look like an access lobe. So long as it is not reversed or changed place. All of the above can be lenient since most allow.

If additional lobes were added to their number: if the extra lobe was on the side of the lobes, or on the front of the lung, which is opposite the heart, it is permitted. But if it was on its back, which is opposite the ribs, it is a tereifah. This applies provided that it is the size of a myrtle leaf (aleh hadas) after the lung has been inflated; but if it is less than this, it is valid. Rema: There are those who are stringent to forbid any extra lobe that is not in the normal row of lobes (bederei de’unin) (Ashiri, Mordechai, Smak, Mahari”v, Tur in the name of the Itur, and others), and so is the practice in some places—to forbid an extra lobe even from the front, without distinguishing between front and back, unless it stands in the normal row of lobes. If, when it is inflated, it inclines toward the front, it is valid; if it inclines toward the back, it is a tereifah (Mahari”v; Beit Yosef in the name of M.K.). In these regions, however, the custom is to permit an extra lobe from the front, and the custom should not be changed.
