Se’if 1א׳

עֲבֵרוֹת שֶׁבֵּין אָדָם לַחֲבֵרוֹ אֵין יוֹם הַכִּפּוּרִים מְכַפֵּר עַד שֶׁיְּפַיְּסֶנּוּ; אֲפִלּוּ לֹא הִקְנִיטוֹ אֶלָּא בִּדְבָרִים, צָרִיךְ לְפַיְּסוֹ; וְאִם אֵינוֹ מִתְפַּיֵּס בָּרִאשׁוֹנָה, יַחֲזֹר וְיֵלֵךְ פַּעַם שְׁנִיָּה וּשְׁלִישִׁית, וּבְכָל פַּעַם יִקַּח עִמּוֹ שְׁלֹשָׁה אֲנָשִׁים, וְאִם אֵינוֹ מִתְפַּיֵּס בִּשְׁלֹשָׁה פְּעָמִים אֵינוֹ זָקוּק לוֹ. מִיהוּ יֹאמַר אַחַר כָּךְ לִפְנֵי י' שֶׁבִּקֵּשׁ מִמֶּנּוּ מְחִילָה (מָרְדְּכַי דְּיוֹמָא וּמַהֲרִי"ל); וְאִם הוּא רַבּוֹ, צָרִיךְ לֵילֵךְ לוֹ כַּמָּה פְּעָמִים עַד שֶׁיִּתְפַּיֵּס. הַגָּה: וְהַמּוֹחֵל לֹא יִהְיֶה אַכְזָרִי מִלִּמְחֹל (מַהֲרִי"ל), אִם לֹא שֶׁמְּכַוֵּן לְטוֹבַת הַמְבַקֵּשׁ מְחִילָה (גְּמָרָא דְּיוֹמָא); וְאִם הוֹצִיא עָלָיו שֵׁם רַע, אֵינוֹ צָרִיךְ לִמְחֹל לוֹ. (מָרְדְּכַי וּסְמַ"ג וְהגה"מ פ"ב מֵהִלְכוֹת תְּשׁוּבָה וּמהרי"ו).

To Reconcile with Your Friend on Erev Yom Kippur, 4 Seifim: Transgressions between people are not subject to atonement on Yom Kippur unless the offender appeases the offended party. Even if one aggrieved another with words alone, this appeasement is necessary. If one cannot effect appeasement at first, one must return a second and a third time, taking along three people each time. If the offended party will not be appeased after three visits, one may desist. (afterwards, however, the offender should relate before ten people that forgiveness was sought) (Mordechai on Yoma, and Maharil). If he is one's Rabbi, one must go to him several times until the Rabbi is appeased. Rema: one should not be cruel and withhold forgiveness (Maharil), unless it is for the benefit of the one seeking forgiveness (Gemara Yoma). If one was a victim of slander, one need not forgive. (Mordechai and Smag and Hagahot Maimuni, Teshuvah chapter 2, and Mahariv).

Se’if 2ב׳

אִם מֵת אֲשֶׁר חָטָא לוֹ, מֵבִיא י' בְּנֵי אָדָם וּמַעֲמִידָם עַל קִבְרוֹ וְאוֹמֵר: חָטָאתִי לֶאֱלֹהֵי יִשְׂרָאֵל וְלִפְלוֹנִי זֶה שֶׁחָטָאתִי לוֹ, וְנָהֲגוּ לְבַקֵּשׁ מְחִילָה בְּעֶרֶב יוֹם כִּפּוּר (מָרְדְּכַי דְּיוֹמָא).

If a man against whom he sinned died, he brings ten men and lets them stand on his grave and he says, “I have sinned against the God of Israel, and against this person who I sinned against him,” (and it was customary to seek forgiveness on the Eve of Yom Kippur)(Mordekhai Yoma)

Se’if 3ג׳

תַּקָּנַת קַדְמוֹנֵינוּ וְחֵרֶם, שֶׁלֹּא לְהוֹצִיא שֵׁם רַע עַל הַמֵּתִים.

It is an enactment of our early authorities, and a ban, that one should not bring out an evil name upon the dead.
