סֵדֶר הַיּוֹם, עַרְבִית וְשַׁחֲרִית וּמִנְחָה אוֹמֵר שָׁלֹשׁ רִאשׁוֹנוֹת, וְשָׁלֹשׁ אַחֲרוֹנוֹת, וּקְדֻשַּׁת הַיּוֹם בָּאֶמְצַע: אַתָּה בְחַרְתָּנוּ וְכו' וַתִּתֶּן לָנוּ ה' אֱלֹהֵינוּ אֶת יוֹם חַג הַמַּצּוֹת הַזֶּה אֶת יוֹם טוֹב מִקְרָא קֹדֶשׁ הַזֶּה, וְע"ל סִימָן ת"צ סָעִיף ג', זְמַן חֵרוּתֵנוּ, יַעֲלֶה וְיָבֹא, וְהַשִּׂיאֵנוּ וְחוֹתֵם: מְקַדֵּשׁ יִשְׂרָאֵל וְהַזְּמַנִּים; וְאִם אָמַר: מְקַדֵּשׁ הַשַּׁבָּת, וְחָזַר בּוֹ תּוֹךְ כְּדֵי דִּבּוּר, יָצָא אַחֲרֵי שֶׁהוּא יוֹדֵעַ שֶׁהוּא יוֹם טוֹב. וְאִם חָל בְּשַׁבָּת אוֹמֵר: אֶת יוֹם הַמָּנוֹחַ הַזֶּה וְאֶת יוֹם חַג הַמַּצּוֹת הַזֶּה, וְחוֹתֵם: מְקַדֵּשׁ הַשַּׁבָּת וְיִשְׂרָאֵל וְהַזְּמַנִּים; וְאֵין אוֹמְרִים בִּרְכַּת מֵעֵין שֶׁבַע.

שָׁכַח לוֹמַר אַתָּה בְחַרְתָּנוּ וְאָמַר יַעֲלֶה וְיָבֹא, יָצָא. הַגָּה: וַאֲפִלּוּ הָיָה שַׁבָּת, אִם הִזְכִּיר יַעֲלֶה וְיָבֹא, יָצָא (בֵּית יוֹסֵף בְּשֵׁם א"ח). וְאִם אָמַר אַתָּה בְחַרְתָּנוּ וְהִזְכִּיר בּוֹ שַׁבָּת, אֲפִלּוּ הָכֵי צָרִיךְ לַחֲזֹר וּלְהַזְכִּירוֹ בְּיַעֲלֶה וְיָבֹא (תְּשׁוּבַת מַהֲרִי"ל); מִיהוּ אִם לֹא הִזְכִּירוֹ בְּיַעֲלֶה וְיָבֹא, אֵין צָרִיךְ לַחֲזֹר (בֵּית יוֹסֵף). וּשְׁלִיחַ צִבּוּר שֶׁשָּׁכַח לְהַזְכִּיר שֶׁל יוֹם טוֹב בְּשַׁחֲרִית, עַיִּןִ לְעֵיל סִימָן קכ"ו.

If one forgot to say Ata Bechartanu (the special holiday blessing) and instead said Ya'aleh V'Yavo (the insertion for festivals), he has fulfilled his obligation. Gloss: Even if it was Shabbat, if he mentioned Ya'aleh V'Yavo, he has fulfilled his obligation (Beit Yosef in the name of Orchot Chaim). However, if he said Ata Bechartanu and included Shabbat within it, he still needs to go back and mention the festival specifically in Ya'aleh V'Yavo (Responsa of Mahari”l). Nonetheless, if he did not mention the festival in Ya'aleh V'Yavo, he does not need to repeat the prayer (Beit Yosef).
