Every place this Name stands — 2,598 occurrences in the Tanakh, in canonical order, first appearance to last. These are the words the lexicon carries under this Name, counting every form the text writes it in — with a particle fused to the front, or bound to what follows — which is the number on the list.
The Name of the first verse of the Torah, and the Name under which God creates and judges: the tradition reads Elohim as the measure of judgement and the four-letter Name as the measure of mercy. Its form is plural and its verb is singular. The Rambam's list names it, and the possessive Elohai — my God — alongside it.

Now before this, Eliashib the priest, who was appointed over the chambers of the house of our God, being allied unto Tobiah,
וָאָבוֹא לִֽירוּשָׁלִָם וָאָבִינָה בָרָעָה אֲשֶׁר עָשָׂה אֶלְיָשִׁיב לְטוֹבִיָּה לַעֲשׂוֹת לוֹ נִשְׁכָּה בְּחַצְרֵי בֵּית הָאֱלֹהִֽים
and I came to Jerusalem, and understood the evil that Eliashib had done for Tobiah, in preparing him a chamber in the courts of the house of God.

And I contended with them, and cursed them, and smote certain of them, and plucked off their hair, and made them swear by God: ‘Ye shall not give your daughters unto their sons, nor take their daughters for your sons, or for yourselves.

The Name is written bound to what follows it, or to whom it belongs, far more often than it is written bare: the God of Avraham, your God, our God, and the form that carries the article.
